I think we all waste far too much intellectual energy on Apple. Everyone has an iPhone and the new ones are simply not enticing enough. It&#8217;s about a totally dysfunctional family racked by drug addiction &#8212; the mother on morphine, the father on alcohol.\u00a0 Not a good play for me currently obsessing on all those years I spent on Prilosec. To make matters worse, my readers bombarding me with &#8220;miracle&#8221;dietary supplements that replace real drugs with mashed up twigs and weeds.<\/p>\n<p style=\"text-align: left;\">Give up pills, dietary supplements. Live a healthy life with exercise and sleep, etc.<\/p>\n<p style=\"text-align: left;\">The Dietary Guidelines for Americans (according to the Federal Government) describes a healthy eating pattern as one that:<\/p>\n<p style=\"padding-left: 30px; text-align: left;\">Includes a variety of vegetables, fruits, whole grains, fat-free or low-fat milk and milk products, and oils. Whole grain products and certain fruits and vegetables like broccoli, potatoes, grape juice, and oranges are sources of chromium (an important chemical for the body). Ready-to-eat bran cereals can also be a relatively good source of chromium.<\/p>\n<p style=\"padding-left: 30px; text-align: left;\">Includes a variety of protein foods, including seafood, lean meats and poultry, eggs, legumes (beans and peas), nuts, seeds, and soy products. Lean beef, oysters, eggs, and turkey are sources of chromium.<\/p>\n<p style=\"padding-left: 30px; text-align: left;\">Limits saturated and trans fats, added sugars, and sodium and\u00a0 Stays within your daily calorie needs.<\/p>\n<p style=\"text-align: left;\">To bore you even further, here&#8217;s an important &#8220;<em>Personal Health<\/em>&#8221; piece from yesterday&#8217;s New York Times. It describes our pill-popping society. If you don&#8217;t want to read it all, skip to the parts I bolded:<\/p>\n<p style=\"padding-left: 30px; text-align: left;\"><strong>The Dangers of `Polypharmacy,&#8217; the Ever-Mounting Pile of Pills<\/strong><\/p>\n<p style=\"padding-left: 30px; text-align: left;\">Dr. Caleb Alexander knows how easily older people can fall into so-called polypharmacy. Perhaps a patient, like most seniors, sees several specialists who write or renew prescriptions.<\/p>\n<p style=\"padding-left: 30px; text-align: left;\">\u201cA cardiologist puts someone on good, evidence-based medications for his heart,\u201d said Dr. Alexander, co-director of the <a href=\"http:\/\/www.jhsph.edu\/research\/centers-and-institutes\/center-for-drug-safety-and-effectiveness\/\" target=\"_blank\">Johns Hopkins Center for Drug Safety and Effectiveness<\/a>. \u201cAn endocrinologist does the same for his bones.\u201d<\/p>\n<p style=\"padding-left: 30px; text-align: left;\">And let\u2019s say the patient, like many older adults, also uses an over-the-counter reflux drug and <a href=\"http:\/\/annals.org\/article.aspx?articleid=2513179\" target=\"_blank\">takes a daily aspirin<\/a> or a <a title=\"In-depth reference and news articles about Zinc.\" href=\"http:\/\/health.nytimes.com\/health\/guides\/poison\/zinc\/overview.html?inline=nyt-classifier\" target=\"_blank\">zinc<\/a> supplement and fish oil capsules.<\/p>\n<p style=\"padding-left: 30px; text-align: left;\">\u201cPretty soon, you have an 82-year-old man who\u2019s on 14 medications,\u201d Dr. Alexander said, barely exaggerating.<\/p>\n<p style=\"padding-left: 30px; text-align: left;\">Geriatricians and researchers have warned for years about the potential hazards of polypharmacy, usually defined as taking five or more drugs concurrently. Yet it continues to rise in all age groups, reaching disturbingly high levels among older adults.<\/p>\n<p id=\"story-continues-1\" style=\"padding-left: 30px; text-align: left;\">\u201cIt\u2019s as perennial as the grass,\u201d Dr. Alexander said. \u201cThe average senior is taking more medicines than ever before.\u201d<\/p>\n<p style=\"padding-left: 30px; text-align: left;\">Tracking prescription drug use from 1999 to 2012 through a large national survey, Harvard researchers reported in November that 39 percent of those over age 65 <a href=\"http:\/\/jama.jamanetwork.com\/article.aspx?articleid=2467552\" target=\"_blank\">now use five or more medications<\/a> \u2014 a 70 percent increase in polypharmacy over 12 years.<\/p>\n<p id=\"story-continues-3\" style=\"padding-left: 30px; text-align: left;\">Lots of factors probably contributed, including the introduction of <a title=\"Recent and archival health news about Medicare.\" href=\"http:\/\/topics.nytimes.com\/top\/news\/health\/diseasesconditionsandhealthtopics\/medicare\/index.html?inline=nyt-classifier\" target=\"_blank\">Medicare<\/a> Part D drug coverage in 2006 and treatment guidelines that (controversially) <a href=\"http:\/\/content.onlinejacc.org\/article.aspx?articleid=1879710#tab1\" target=\"_blank\">call for greater use of statins<\/a>.<\/p>\n<p style=\"padding-left: 30px; text-align: left;\">But older people don\u2019t take just prescription drugs. An article published in JAMA Internal Medicine , using a longitudinal national survey of people 62 to 85, may have revealed the fuller picture.<\/p>\n<p style=\"padding-left: 30px; text-align: left;\">More than a third <a href=\"http:\/\/archinte.jamanetwork.com\/article.aspx?articleid=2500064&amp;resultClick=3\" target=\"_blank\">were taking at least five prescription medications<\/a>, and almost two-thirds were using <a title=\"Recent and archival health news about dietary supplements and herbal remedies.\" href=\"http:\/\/topics.nytimes.com\/top\/news\/health\/diseasesconditionsandhealthtopics\/dietarysupplementsandherbalremedies\/index.html?inline=nyt-classifier\" target=\"_blank\">dietary supplements<\/a>, including herbs and <a title=\"In-depth reference and news articles about Vitamins.\" href=\"http:\/\/health.nytimes.com\/health\/guides\/nutrition\/vitamins\/overview.html?inline=nyt-classifier\" target=\"_blank\">vitamins<\/a>. Nearly 40 percent took over-the-counter drugs.<\/p>\n<p style=\"padding-left: 30px; text-align: left;\">Not all are imperiled by polypharmacy, of course. But some of those products, even those that sound natural and are available at health food stores, interact with others and can cause dangerous side effects.<\/p>\n<p style=\"padding-left: 30px; text-align: left;\">How often does that happen? The researchers, analyzing the drugs and supplements taken, calculated that more than 8 percent of older adults in 2005 and 2006 were at risk for a major drug interaction. Five years later, the proportion exceeded 15 percent.<\/p>\n<p style=\"padding-left: 30px; text-align: left;\">\u201cWe\u2019re not paying attention to the interactions and safety of multiple medications,\u201d said Dima Qato, the lead author of the JAMA Internal Medicine article (Dr. Alexander was a co-author) and a pharmacist and epidemiologist at the University of Illinois at Chicago. \u201cThis is a major public health problem.\u201d<\/p>\n<p id=\"story-continues-4\" style=\"padding-left: 30px; text-align: left;\">She was stunned to discover, for instance, that the use of <a href=\"http:\/\/www.mayoclinic.org\/drugs-supplements\/omega-3-fatty-acids-fish-oil-alpha-linolenic-acid\/background\/hrb-20059372\" target=\"_blank\">omega-<\/a><a href=\"http:\/\/www.mayoclinic.org\/drugs-supplements\/omega-3-fatty-acids-fish-oil-alpha-linolenic-acid\/background\/hrb-20059372\" target=\"_blank\">3 fish oil supplements<\/a> had quadrupled over five years. Her research suggests that almost one in five older adults now takes them.<\/p>\n<p style=\"padding-left: 30px; text-align: left;\">Users probably believe fish oil helps their hearts. But Dr. Qato pointed out fish oil capsules lacked regulation and <a href=\"http:\/\/jama.jamanetwork.com\/article.aspx?articleid=1357266\" target=\"_blank\">evidence of effectiveness<\/a>, and can cause <a title=\"In-depth reference and news articles about Bleeding.\" href=\"http:\/\/health.nytimes.com\/health\/guides\/injury\/bleeding\/overview.html?inline=nyt-classifier\" target=\"_blank\">bleeding<\/a> in patients taking blood thinners like <a href=\"http:\/\/www.webmd.com\/drugs\/2\/drug-3949\/warfarin-oral\/details\" target=\"_blank\">warfarin<\/a> (brand name: Coumadin).<\/p>\n<p style=\"padding-left: 30px; text-align: left;\">Though drug interactions can occur in any age group, older people are more vulnerable, said Dr. Michael A. Steinman, a geriatrician at the University of California, San Francisco, who <a href=\"http:\/\/archinte.jamanetwork.com\/article.aspx?articleid=2500058\" target=\"_blank\">wrote an accompanying commentary<\/a>.<\/p>\n<p style=\"padding-left: 30px; text-align: left;\">Most have multiple chronic diseases, so they take more drugs, putting them at higher risk for threatening interactions.<\/p>\n<p style=\"padding-left: 30px; text-align: left;\">The consequences can also be more threatening. Say a drug makes older patients dizzy.<\/p>\n<p style=\"padding-left: 30px; text-align: left;\">\u201cThey\u2019re more prone to fall, because they don\u2019t have the same reserves of balance and strength\u201d as the young or middle-aged, Dr. Steinman said. \u201cAnd if they do fall because they\u2019re dizzy, they\u2019re more likely to get hurt.\u201d<\/p>\n<p style=\"padding-left: 30px; text-align: left;\">Some common combinations that cropped up in the study and could spell trouble: aspirin and the anti-clotting drug <a href=\"http:\/\/www.webmd.com\/drugs\/2\/drug-5190\/clopidogrel-oral\/details\" target=\"_blank\">clopidogrel<\/a> (Plavix), both blood thinners that together increase the risk of bleeding with long-term use; aspirin and <a href=\"http:\/\/www.mayoclinic.org\/drugs-supplements\/naproxen-oral-route\/description\/drg-20069820\" target=\"_blank\">naproxen<\/a> (<a title=\"Recent and archival health news about Aleve.\" href=\"http:\/\/topics.nytimes.com\/top\/news\/health\/diseasesconditionsandhealthtopics\/aleve_drug\/index.html?inline=nyt-classifier\" target=\"_blank\">Aleve<\/a>), over-the-counter drugs that when combined can cause bleeding, ulceration or perforation of the stomach lining.<\/p>\n<p style=\"padding-left: 30px; text-align: left;\">Dr. Qato recalled reviewing the medications of a 67-year-old man taking both the <a title=\"In-depth reference and news articles about Cholesterol.\" href=\"http:\/\/health.nytimes.com\/health\/guides\/nutrition\/cholesterol\/overview.html?inline=nyt-classifier\" target=\"_blank\">cholesterol<\/a> drug <a href=\"http:\/\/www.webmd.com\/drugs\/2\/drug-6105\/simvastatin-oral\/details\" target=\"_blank\">simvastatin<\/a> (<a title=\"Recent and archival health news about Zocor.\" href=\"http:\/\/topics.nytimes.com\/top\/news\/health\/diseasesconditionsandhealthtopics\/zocor_drug\/index.html?inline=nyt-classifier\" target=\"_blank\">Zocor<\/a>) and the <a title=\"In-depth reference and news articles about Blood Pressure.\" href=\"http:\/\/health.nytimes.com\/health\/guides\/test\/blood-pressure\/overview.html?inline=nyt-classifier\" target=\"_blank\">blood pressure<\/a> medication <a href=\"http:\/\/www.webmd.com\/drugs\/2\/drug-5891\/amlodipine-oral\/details\" target=\"_blank\">amlodipine<\/a> (Norvasc) \u2014 the most common combination of interacting drugs that emerged in her study.<\/p>\n<p style=\"padding-left: 30px; text-align: left;\"><a title=\"Recent and archival health news about statins (cholesterol-lowering drugs).\" href=\"http:\/\/topics.nytimes.com\/top\/news\/health\/diseasesconditionsandhealthtopics\/statins_cholesterollowering_drugs\/index.html?inline=nyt-classifier\" target=\"_blank\">Statins<\/a>, along with their cholesterol-lowering properties, can cause <a title=\"In-depth reference and news articles about Muscle aches.\" href=\"http:\/\/health.nytimes.com\/health\/guides\/symptoms\/muscle-aches\/overview.html?inline=nyt-classifier\" target=\"_blank\">muscle pain<\/a> and weakness; Norvasc heightens that risk. A different blood pressure drug \u2014 there are many alternatives \u2014 would be a safer choice, Dr. Qato said. Yet almost 4 percent of the older adults in her study took both drugs.<\/p>\n<p id=\"story-continues-5\" style=\"padding-left: 30px; text-align: left;\">Moreover, though her patient wasn\u2019t experiencing problems, he was also taking garlic and omega-3 supplements, which can interact with prescription medications.<\/p>\n<p style=\"padding-left: 30px; text-align: left;\">\u201cDid you tell your doctor you were on them?\u2019\u201d Dr. Qato recalled asking. \u201cHe said, \u2018No, why should I? If it was important, why didn\u2019t he ask me?\u2019\u201d<\/p>\n<p style=\"padding-left: 30px; text-align: left;\">A reasonable question. A recent study in JAMA Internal Medicine, however, found that more than 42 percent of adults <a href=\"http:\/\/archinte.jamanetwork.com\/article.aspx?articleid=2500061\" target=\"_blank\">didn\u2019t tell their primary care doctors<\/a> about their most commonly used complementary and alternative medicines, including a quarter of those who relied most on herbs and supplements.<\/p>\n<p style=\"padding-left: 30px; text-align: left;\">Usually, that was because the physicians didn\u2019t ask and the patients didn\u2019t think they needed to know; in a few cases, doctors had previously discouraged alternative therapies, or patients thought they would.<\/p>\n<p style=\"padding-left: 30px; text-align: left;\">And they might, especially for older patients with complex regimens. \u201cI\u2019m not a big fan of supplements,\u201d Dr. Alexander tells patients taking lots of vitamins, supplements and herbal remedies.<\/p>\n<p style=\"padding-left: 30px; text-align: left;\"><strong>\u201cI think the vast majority of evidence raises serious questions about their effectiveness or, in some cases, their safety. They\u2019re less well regulated than prescription medications. I think you\u2019d be better off stopping them.\u201d<\/strong><\/p>\n<p style=\"padding-left: 30px; text-align: left;\">Patients often resist, he said, and \u201cthey\u2019re the captain of their own ship.\u201d So he explains <a href=\"http:\/\/annals.org\/article.aspx?articleid=1789253\" target=\"_blank\">the risks and benefits<\/a>, and negotiations ensue.<\/p>\n<p style=\"padding-left: 30px; text-align: left;\">Often, though, patients don\u2019t know that a daily aspirin, Prilosec OTC or fish oil can interact with other drugs. Or they\u2019re confused about what they\u2019re actually taking.<\/p>\n<p id=\"story-continues-6\" style=\"padding-left: 30px; text-align: left;\">Dr. Steinman recalled asking a patient to bring in every pill he took for a so-called brown bag review. He learned that the man had accumulated four or five bottles of the same drug without realizing it, and was ingesting several times the recommended dose.<\/p>\n<p style=\"padding-left: 30px; text-align: left;\">Ultimately, the best way to reduce polypharmacy is to overhaul our fragmented approach to health care. \u201cThe system is not geared to look at a person as a whole, to see how the patterns fit together,\u201d Dr. Steinman said.<\/p>\n<p style=\"padding-left: 30px; text-align: left;\">In the meantime, though, patients and families can ask their physicians for brown bag reviews, including every supplement, and discuss whether to continue or change their regimens. Pharmacists, often underused as information sources, can help coordinate medications, and some patients <a href=\"https:\/\/www.medicare.gov\/part-d\/coverage\/medication-therapy-management\/medication-therapy-programs.html\" target=\"_blank\">qualify for medication reviews through Medicare<\/a>.<\/p>\n<p style=\"padding-left: 30px; text-align: left;\"><strong>\u201cWe spend an awful lot of money and effort trying to figure out when to start medications,\u201d Dr. Alexander said, \u201cand shockingly little on when to stop.\u201d<\/strong><\/p>\n<p style=\"text-align: left;\"><strong><span style=\"color: #0000ff;\">Favorite (?) bank, standard idiocy.
